Pre-Appointment Checklist
To prepare for your scheduled psychiatric care appointment:| Bring a list of your current medications and dosages, medical conditions, and any allergies for Cleveland psychiatrist Dr. Sabet to review. |
| If you have visited your primary care physician, bring a copy of your most recent exam and any lab results. |
| Bring your insurance information, and cash or credit card for payment of your psychiatric evaluation. You can be reimbursed when your insurance pays. |
