Pre-Appointment Checklist
In preparation for yourscheduled psychiatric care appointment please complete the following:
| Bring a complete list of your current medications and dosages, medical conditions, and any allergies for Cleveland psychiatrist Farid Sabet to review. |
| If you have visited your primary care physician, bring a copy of your most recent exam and any lab results. |
| Bring your insurance information, including claims address. Copays are due at the time of your appointment. |
